Your First Visit
CONSULTATION
During your first visit to our office you will be asked to fill
out our required forms which will help us gain some information about
yourself and your condition. You may complete our initial patient paperwork
prior to your visit by clicking on the following download: br>
New Patient Form.
If you do not complete it prior to your visit, please be sure to arrive
20 minutes prior to your appointment so that you have time to complete it.
Here are some helpful hints in filling out our paperwork:
1. Be sure to complete each page.
2. If you do not have a "Secondary Complaint" that page may be left blank.
3. When completing your history please write N/A if it does not apply to you.
The doctor will then talk with you and ask some
specific questions regarding your condition to determine if chiropractic
care may be of benefit to you. If your doctor can help you, he will let
you know right away. If your doctor cannot help you, he will also tell
you that right away and you will be referred to a doctor that will be
able to help you the most. Nothing will be done in our office without
your consent.
EXAMINATION
After a thorough consultation with the doctor, you will have a
complete chiropractic examination. Depending on your reason(s) for
seeking our care, your evaluation may include: range of motion testing,
standard orthopedic testing, neurological testing (reflexes, motor
strength/weakness), and outcome measures that allow us to monitor your
condition and progress during care.
The doctor will gather valuable information
regarding your condition so that he can gain a better understanding of
how chiropractic can help you. If necessary, the doctor may refer you
for additional testing procedures (i.e. x-ray, MRI).
Once all the information and examinations have
been performed, the doctor will give you a report of findings and tell
you recommendations for treatment.
TREATMENT
Most of our patients begin to see results from the very first
adjustment. Our unique, gentle yet effective adjustments are able to
restore function and movement for people of every age.
